We are proud of what we have done. Here’s some of the achievements we would like to share.
We have provided high quality essential medical services
SHM provides a range of primary care services (General Practice, nursing, etc.) across 3 sites: Charlestown and Lower Kersal, Little Hulton, and Eccles, to 13,000 patients. We have significantly improved the quality of these services, with Quality and Outcomes Framework (QOF) scores increased from 53% to 93% over 18 months.
We have improved the access to primary care, with increased number of appointments, increased appointment times, and the first practice in Salford to offer extended opening hours (early mornings, evenings and Saturdays).
We have developed an excellent clinical team with a broad skill mix of salaried GPs, Advanced Practitioners, Practice Nurses, Assistant Practitioners, together with the wider primary care team. The team offer a range of interventions, from individual and group sessions for preventative health input, through to Proactive management of long term conditions to improve quality of life for patients and prevent unnecessary admissions.
We have expanded services
SHM provide a range of enhanced services using the range of specialisms within our experienced clinical team, for example sexual health, epilepsy, dermatology, child health surveillance and minor surgery. For example, we secured Practice Based Commissioning funds to provide physiotherapy at accessible sites across the city to help people either move into or retain employment. We also provides access to primary health care services for homeless people in Salford, through key access points across the city.
We have developed a learning organisation
SHM put together an annual programme of staff training and development, and has created a range of career development and leadership opportunities for our staff.
We now have a track record in health care worker training, with strong links with Manchester University Medical School, the North West Deanery, the University of Bolton and Salford PCT. Our programme includes undergraduate medical students, postgraduate Doctors, Nursing undergraduates, postgraduate Nurses, and healthcare assistants. We help refugee health professionals retrain to practice in the UK. We also offer placements to local young people within our admin teams.
We have created exciting social enterprise opportunities
Small changes have big effects in communities. We have supported programmes for healthy nutrition, exercise, social support and personal development our communities. For example, we have set-up a social prescribing service, Refresh, that supports people to access a wide range of non-medical support to address their health needs, and develops new opportunities within local communities, such as an allotment, laughter workshops, and art classes.
But that’s still not good enough – we can do even better!
