Salford Health Matters has just received £400,000 over 3 years from the Department of Health to help improve health services in the community for children. Our Salford Children's Community Partnership provides an exciting opportunity for two new exciting nursing roles to lead the transformation children’s acute care services in the community.
This ground-breaking project is funded by the Department of Health and is designed to substantially reduce children’s emergency admissions to hospital and improve the quality of care for acutely unwell children by putting high level children’s expertise (an APNP and a Community Children’s Nurse) in the community to keep children at home whenever possible.
The project will be delivered in collaboration with Hope Street Centre for health service development, who will evaluate the project and Kids Health Matters CIC, a new social enterprise looking to revolutionise community based paediatrics and take this model of care national.
We are seeking an Advanced Paediatric Nurse Practitioner (APNP) and a Community Children’s Nurse (CCN) to take this innovative project forward.You will work with the Salford Health Matters team to design and implement the new service and its associated evaluation. Key deliverables for the project include better health and well-being outcomes for Salford children and their families, improved integrated working within the community setting and a significant reduction in emergency paediatric admissions.
For both posts we are looking for a motivated, enthusiastic team player with great interpersonal skills and a passion for their work.
